- If you have had an appointment with one of our providers recently, you will receive an automatic invite sent via e-mail if:
- Your GWMFA patient account has a valid email address on file
- You are not currently a FMH member
- You have not rejected an FMH invitation in the past
- Patients are unable to sign up for FMH on their own. Automatic invites go out each night.
- If you do not find the invitation email in your Inbox, please check your Spam or Junk folder.
- NOTE: Some email providers, such as those associated with work/government, employ strict filtering methods. Update your email address with our Front Desk staff when you visit any of our clinic locations.
- You will be required to enter an INVITATION CODE, which will be given to you in your email. Please note: the invitation code is only active for 30 days.
